Blue Heart Community Care is NDIS registered service provider that has been providing services since 2020, which provides support to people with disability in their day-to-day life activities and supported independent living. Blue heart community care provides people with disability highest quality support to live their life the way they want by empowering them to make their life choices.

ACCOMMODATION AND ASSISTANCE AVAILABLE NOW

24/7 support is provided by staff our qualified support workers in supporting people with mental health-related disability, intellectual disability, and autism spectrum disorder. We are also able to support participants with hospital Discharge before returning to their homes We help our clients with daily life tasks and develop your skills for independent living. Support will be tailored to helping you achieve the goals in your Plan.

Staff will be 24/7 in the house with a sleepover model.

The property offers easy access to public transport and is walking distance to shopping centre and community centres.

The medical centre is located closed o the premises.

We also provide community access, transportation, recreational activities to our clients.

we don't have waiting period, can take clients on board with in 24 hours.
