Blue Heart Community Care are an NDIS registered service provider that have been providing services since 2020. We provide support to people with disability in their day-to-day life activities and supported independent living. Blue heart community care provides people with disability highest quality support to live their life the way they want by empowering them to make their life choices.

ACCOMMODATION AND ASSISTANCE AVAILABLE NOW

24/7 support is provided to participants by our qualified support workers for those living with with physical, complex, mental health-related, and intellectual disabilities Capacity to support participants with Rapid Hospital Discharge before returning to their homes, or transitioning to one of our properties We assist our participants with daily life tasks and development of their skills for independent living Support will be tailored to assisting participants achieve their NDIS goals

Staff will be 24/7 in the house with a sleepover and active nights model.

The property offers easy access to public transport and is walking distance to Casey Filed, Casey race and community centres.

The medical centre is located closed o the premises.

We also provide community access, transportation, recreational activities to our clients.

No waiting period, and have capacity to accommodate participants with in 24 hours.
